Mail Merge Questions

I am using Ubuntu 15.10 and LO Version: 5.0.5.2

I am attempting to use Mail Merge from a db to format a book that I am creating. First, a questions regarding mail merge:

1) is there a way to create the merged document with all of the records on a single page (or continuous pages as necessary) rather than having merge create a page for each record?

I have tried generating the TOC two ways;

first by creating the TOC before running mail merge – this resulted in the TOC being generated with the field names from the db.

second by creating the TOC after running the mail merge – the TOC was created by only with the field names from the db.
